registerNew Patient Registration

Patients wishing to register with the Practice should complete a registration form and attend a New Patient Check appointment with the nurse. For children and incapable adults the form should be signed by a parent or other authorised adult. Patients who are accepted onto the Practice list will be given a health questionnaire for completion and invited to participate in a health check.

Please make an appointment for a new patient health check and bring the completed forms with you to that appointment. The Practice may also temporarily register patients who are residing in the Practice area on a temporary basis for a period of more than 24 hours and up to 3 months.

If the Practice refuses an application by a patient to be included on the practice list, or an application for inclusion as a temporary resident, or if the practice has terminated responsibility for providing services to a patient, the Local Health Board is responsible for commissioning services for those patients.

If a patient is unable to find another practice willing to accept him/her onto its list, the patient should contact: Registration Department NHS Wales Business Services Centre, Floor3 Matrix House, Matrix Park Swansea SA6 8BX  01792 860476. We aim to provide consultations within 24 hours of Registration.

Patients who move outside the Practice area will be required to seek the agreement of the Practice to remain on the list and let the Local Health Board know that such an agreement has been reached with the Practice. Failure to do so may result in removal from the list.
